If you are unable to fill out the forms prior to your appointment, we ask that you arrive 15-20 minutes early on the day of your scheduled appointment to complete the New Patient paperwork.
If you have a medical condition, which requires premedication, please call or text our office and we will send in a prescription to your pharmacy for pick up prior to your appointment.
